I created this health binder for my own family after realizing that my husband wouldn't know where to start if he needed our son's medical information in an emergency. I wanted something simple — no apps, no subscriptions, no 40-page workbook. Just the essentials, organized and printed.

Once I had it set up, I realized every family could use one. So I turned it into a template you can fill in and have done in an afternoon.

What format is the template in?
It's a Google Doc. After purchase, you'll receive a view-only link. Just click "File → Make a Copy" to save your own editable version to your Google Drive. You can then fill it in on your computer, tablet, or phone — and print when you're ready.
Can I use this for my whole family?
Yes! Make as many copies as you need — one per family member. The template works for adults and can easily be adapted for kids by swapping sections (like replacing "Pregnancy History" with birth details for little ones).
Do I need to update it constantly?
Nope — that's the beauty of it. You fill it in once and only update when something actually changes: a new doctor, a new diagnosis, a new medication. It's a reference document, not a journal.
What about insurance information?
Insurance was intentionally left out because it changes frequently (often annually). Instead, just slip a photocopy of your insurance card into a sheet protector in your binder — easy to swap when your plan changes.
